Obi-Wan manages to open the plates on his opponent's chest, gaining access to his internal organs. Grievous goes berserk and throws him to the edge of the platform, grabbing an electrostaff to kill the Jedi. Thoth, exhausted from the fight and only able to hold on to the edge of the platform, uses the Force to draw his rifle.

He fires five shots at Grievous' body. The General's organs light up, and he begins to burn from the inside. Fire bursts from the eye sockets of his mask, then he finally falls dead.

There are 2 scenario explanations for why Grievous has lung problems:

The first reason was that the technology of cyborgization was still imperfect, and his transformation into a cyborg was an important step forward.

The second reason was the damage to Grievous' chest by Mace Windu during the Battle of Coruscant (in the second season of the Clone Wars cartoon) there is a coughing Grievous.

Grievous' cough itself is inspired by George Lucas's cold, who found it interesting to add a new element to the character's image.
